[Choose #5] never had to fill one in, yet.
[Choose #4] No way!! Why would you do that!? Really, I can’t wait to see the results and comments to this question.
[Choose #5] Yes, because I just like to Lie!
[Choose #4] I wouldn’t dream of it.
[Choose #1] Just once, when I was new to diving. I was told that the dive op would not accept any Dr’s Medical Cert, so if I didn’t lie, I wouldn’t be allowed to dive. I haven’t run into that since, and I woldn’t lie again.
[Choose #4] If I’m sick I don’t go diving.
[Choose #5] if you lied why would you admit it?
[Choose #1] Instructor (regarding a broken rib) ended badly
[Choose #3] My doctor cleared me to dive when I was in the worst shape in my life. I didn’t see the need to get a new opinion every year when I was in much better shape.
[Choose #3] need another option, "yes I lied because I understood the repercusions of diving with my issue"
[Choose #4] Why would you place yourself in jeopardy? Be honest and dont make others responsible for you.
[Choose #4] I value my life too much to lie on any medical form and yes it has kept me out of the water sometimes, but I am alive.
[Choose #3] BP issues under control. Big controversy over it

[Choose #4] started diving after I had a major stroke. Had to go to 4 dr.’s but got it signed
[Choose #4] to lie on a medical form not only endangers the diver, it endangers every diver who dives with that diver in the future, any pro who would tell a diver to lie is no professional in my book.
[Choose #5] Not YET. Not out of the question either.
[Choose #4] Thank god for good health (for now)

[Choose #3] It was a no longer pertinent info
[Choose #4] It’s best to honest and get the okay from a doctor than to risk it.
[Choose #4] I never had the need to. I asked my Dr., because of my High Blood presure and he told me if I wanted him to clear me for Diving I had to quit smoking. So I quit smoking and he cleared me.
[Choose #4] never had to !
[Choose #5] no, but got Dr’s release.
[Choose #4] But I’ve never had to fill one out either
[Choose #4] Those who lie are only thinking of themselves, not the dive trip they could ruin for others!
[Choose #5] There are no scuba police and that information is no ones business except for me and my doctor
[Choose #4] I get an annual physical and have my Doctor fill out the medical release. If I’m going anywhere where I’m not known I take a copy with me. I’ve never had any further questions.
[Choose #4] No never.. But it’s also an invasion of privacy..
[Choose #4] Although I wondered if I should answer ’yes’ to PADI’s question "Could you be pregnant, or are you attempting to become pregnant?" just for fun. I guess if I were a male seahorse it would’ve been a valid question.
[Choose #4] Not being honest on a SCUBA medical form places you, your instructor and anyone else involved in the process at risk. Not a good idea.
