Have you ever lied on a scuba diving medical form? Was it because you were persuaded to lie by your scuba instructor or dive professional? Did you lie because you really just wanted to go scuba diving? Did you lie because you felt your medical issue wasn’t that bad. OR do you always tell the truth on scuba diving medical forms?

[Choose #3] No secret amongst my dive buddy’s that I have a bad back. But have fibbed about it on the form when doing a course, but not in person to the instructor.

[Choose #1] Just once, when I was new to diving. I was told that the dive op would not accept any Dr’s Medical Cert, so if I didn’t lie, I wouldn’t be allowed to dive. I haven’t run into that since, and I woldn’t lie again.

[Choose #3] My doctor cleared me to dive when I was in the worst shape in my life. I didn’t see the need to get a new opinion every year when I was in much better shape.

[Choose #4] to lie on a medical form not only endangers the diver, it endangers every diver who dives with that diver in the future, any pro who would tell a diver to lie is no professional in my book.

[Choose #4] I never had the need to. I asked my Dr., because of my High Blood presure and he told me if I wanted him to clear me for Diving I had to quit smoking. So I quit smoking and he cleared me.

[Choose #4] I get an annual physical and have my Doctor fill out the medical release. If I’m going anywhere where I’m not known I take a copy with me. I’ve never had any further questions.
